Medication reminder and health tracking app
Overview
MedRemind was developed for a regional healthcare network seeking to improve medication adherence among patients with chronic conditions. Studies showed that non-adherence to medication regimens was leading to poorer health outcomes, increased hospital readmissions, and higher healthcare costs. Particularly affected were elderly patients and those on complex medication schedules who struggled to keep track of their prescriptions. We created a comprehensive mobile application that combines medication management, health tracking, and healthcare provider communication in one intuitive platform. The app features smart medication reminders with verification, detailed medication information, automated refill notifications, and vital sign tracking. The system synchronizes with pharmacy systems for prescription updates while providing healthcare providers with adherence reports to facilitate better care management. Since its implementation, MedRemind has significantly improved medication adherence rates across the healthcare network, particularly among high-risk patient populations.
